Center of Rehabilitation
When you are recovering from illness, orthopedic surgery, stroke or injury, our Center of Rehabilitation (COR) will provide the care, therapy and support you need to successfully transition from the hospital to home. The COR is conveniently accessible from hospitals in the Fox Cities and Green Bay.
Our licensed nurses and certified nursing assistants along with occupational, physical and speech therapists are available to patients and their physicians for ongoing medical management. A social worker is assigned to the unit to manage admission consultations and discharge planning and to provide family and patient support.
Our pre-registration program is available if your surgery is elective and will require rehabilitation. When you pre-register the benefits are:
- Have your questions answered before your surgery and rehab
- Meet the staff who will partner with your physician ordered rehab plan of care
- Reserve your room so the process of discharging from the hospital is easier
- Complete your paperwork in advance
- Allow the focus to be on your rehabilitation after surgery
We are a preferred provider for most health insurance carriers, including Network Health Plan.
The COR unit has its own entrance and includes eight private rooms and four semi-private rooms all with telephones and televisions. While you are recuperating, you will have individualized meals, scheduled activities and spiritual care opportunities.
Remember, you have a choice in selecting a rehabilitation facility. So choose the COR Unit at St. Paul Elder Services. Together, we can help you get back into daily living.
