Memory Care - Passages
Passages is our Memory Care program that is designed to meet our Elders at whatever point they come to us. Once their needs are identified and assessed by our professional team, our goal is to assist each
Elder to maintain their optimal level of function for as long as possible. This is managed in two home-like and secure neighborhoods:
Appletree Court, housing 18 private-rooms, is located between the Home and Villa. This neighborhood features two enclosed courtyards, an open kitchen and lounge area that enhance familiar routines. Individuals in the early stages of dementia benefit from group programs throughout the day and evening.
Poppy Path provides additional nursing care as the disease progresses with continued emphasis on a social model of support. This neighborhood serves 20 residents in 16 spacious private rooms and two semi-private rooms. Individualized social and recreational programming along with separate dining and lounge areas are included.
Through the use of therapeutic interventions, our goal is to:
- Establish and enhance communication
- Provide comfort
- All choice and self-expression
- Stimulate recognition of people and places that have meaning
- Promote a sense of purpose and belonging
- Create and maintain a feeling of security and home
